Residential Living
If you are interested in submitting an application to the Residential Living Program at Connecticut Baptist Homes, Inc.:

Arrange a Tour: All applicants interested in admission to the Residential Living program are encouraged to take a tour of the apartments as a first step. Please call (203) 237-1206 to schedule a tour.

After the tour, an application will be given to the perspective resident. Upon receipt of the completed application, the admission director will meet with the prospective resident to assess and discuss the application and their needs.

Skilled Nursing Neighborhoods
If you are interested in submitting an application to Connecticut Baptist Homes, Inc. for Skilled Nursing or Intermediate Care:

Submit an application: An application form may be obtained through the admissions office.  Please call (203) 237-1206 or contact us via e-mail.  Admission to a Skilled Nursing or Intermediate Care requires a written application.

Upon receipt of the completed application, a clinical evaluation will be done and the admissions office will call to discuss availability. Please note that at times we are unable to admit at the time of application due to lack of available space. In that case, your name will be put on a wait list and you will be contacted as soon as a room becomes available.   

At the time of admission, we will request orders for admission and a summary of the resident's current condition from the resident's physician.
